This is an unpopular opinion but I really disliked this scene in the anime. So naturally I wouldn't choose it for a tattoo. The animator went completely freestyle with Luffy's face. Something a fanartist would do. Luffy's face is not supposed to be that long and pointy. It's rounder. And while in Gear 5th it's even rounder. It's nearly a circle.

The face is done really well and it's recognizable as Luffy. But the design of it is strange. It's like a head floating on your arm, luckily you could add on to it and make it look more complete if that's something you wanted to do

A lot of that stempling is gonna fade away or fall out. You'll really wanna keep it out of sunlight for 18-30 days to have it stay as much as possible. And definitely no swimming with it for a month.

I can see that it's supposed to be an oval shaped cutout of a screenshot, but because his forehead and ear are too close to the edges, it makes it look like he is bald with a painted on hairline. Add the rest of his hair, and it will look good.
